Upper left panel shows the typical spindle cell histologic appearance of embryonal rhabdomyosarcoma. The lower left panel is a schematic depiction of the normal 11p15 locus around IGF-2 and the LOH at the 11p15 locus typically seen in ERMS. In the normal locus, IGF-2 is expressed from the paternal allele where there is methylation of upstream of the transcriptionally repressed H19 gene. In contrast, the maternal allele is unmethylated in this region leading to transcriptional activation of H19 and transcriptional repression of IGF-2. With LOH, the paternal allele is duplicated leading to biallelic transcription of IGF-2. The upper right panel shows the typical histologic appearance of alveolar rhabdomyosarcoma with round cells arranged in a pattern reminiscent of a pulmonary alveolus. The lower right panel shows the typical t(2;13)(q35;q14) translocation seen in these tumors. The translocation fuses exon 7 of the PAX3 gene to exon 2 of the FKHR gene. The fusion brings together the paired box (PB) and homeodomain (HD) DNA binding regions of PAX3 with the transactivating domain (TAD) of FKHR. The vertical line centered below the horizontal line indicates the fusion break-point junction.
