During T_H2-dominated inflammation (e.g., atopic dermatitis), and probably in certain T_H2-involved tumors (CTCL), as well as probably prurigo, IL-31 is released from activated skin-homing T_H2 cells. IL-31 binds to the IL-31RA receptor (IL31RA/OSMRβ receptor complex) on sensory nerve endings in the skin. As an amplification mechanism, IL-31RA communicates with ion channels such as TRPV1 that result in enhanced Ca signaling, STAT3 activation, and probably other signal transduction pathways in sensory neurons. The transducer of IL-31-mediated itch from central primary afferents to the dorsal horn of spinal cord is currently unknown and probably involves opioids. The role of oncostatin M (OSM) in itch is poorly understood. However, a mutation in familial pruritic amyloidosis, a severely itchy skin disease, has been described for OSMRβ, the receptor for OSM. A receptor cross talk of IL-31RA and TRPV1 may lead to the amplification of neuronal sensitization, enhanced release of neuropeptides (SP, CGRP), and other mediators involved in neurogenic inflammation and itch.
