A mixed lytic and sclerotic destructive lesion with periosteal elevation and a soft tissue mass. This lesion is very characteristic of a malignant lesion on radiographs. However, multiple biopsies were originally thought to represent benign eosinophilic granuloma in this 14-year-old male. After the fifth biopsy this was eventually diagnosed as primary Hodgkin’s disease of bone. At the time of staging laparotomy, the patient was noted to have regional lymph node involvement as well as splenic involvement. In the latter areas, the histopathology showed characteristic Reed-Sternberg giant cells that were not present in the primary lesional area that consisted of mostly a lymphocytic, a histiocytic, and an eosinophilic infiltrate. Initially, a diagnosis of eosinophilic granuloma was entertained; however, on repeat biopsies, an increasing amount of sclerosis or collagen formation was noted, leading to the diagnosis of nodular sclerosing Hodgkin’s disease, primary of bone.

Histiocytosis Microscopy. Electron microscopy further aids in the diagnosis by identifying Birbeck granules within Langerhans cells. These granules are "tennis-racket-shaped" cytoplasmic inclusions with a zipper-like appearance, believed to play a role in antigen storage. Contributed by S Bhimji, MD
