Although rates of thrombectomy and embolectomy were highly variable, there was no consistent geographic pattern. Forty-two hospital referral regions had surgery rates at least 30% above the national average; 51 regions had rates more than 25% below average.

Pharmacomechanical treatment of a thrombosed graft. (a) A very small amount of contrast was gently injected through the infusion catheter to illustrate the residual clot (arrowhead) along the graft after an initial pulse spray (not shown) with a total of 4 mg of tissue plasminogen activator. Crisscrossing sheaths (arrows) are seen. (b) The outflow is improved by treating the underlying stenosis in the venous anastomosis of the graft. Note the indentation that the focal lesion makes on the angioplasty balloon. (c) A suboptimal initial result required repeated and prolonged percutaneous transluminal angioplasty using high pressure, as well as subsequent balloon upsizing. (d) Passage of the percutaneous thrombectomy device (rotating basket, arrow) allowed further clearance of clots and mural thrombi. Following this, a Fogarty embolectomy balloon was passed into the brachial artery to pull back the platelet plug at the arterial anastomosis of the graft (not shown). (e) A typical procedural result for graft venous intimal hyperplasia, with re-establishment of good graft flow.
