FAB-L2 blasts are generally larger and more heterogeneous in size and shape than FAB-L1. The nucleoli are more numerous and prominent. A variable amount of lightly basophilic agranular cytoplasm is seen. Diagnostic confusion may occur with poorly differentiated myeloid leukemias or megakaryocytic leukemia, which can be resolved with histochemical stains, Tdt assay, or immunophenotyping.
