Drugs in overactive bladder syndrome

Overactive bladder syndrome is characterised by a need to rush to urinate ‐ urine is passed frequently and there may be incontinence. The main treatment option is drug therapy. The most commonly used drugs are anticholinergics, but these often have side‐effects, such as dry mouth. This review sought evidence to compare other types of drugs with anticholinergics. Only a few, small‐scale randomised trials were found, many testing drugs that are no longer used clinically. The review found inadequate evidence to assess whether or not available alternative drugs are better or worse than anticholinergics in the management of people with symptoms of overactive bladder syndrome.

Drug Class Review: Agents for Overactive Bladder: Final Report Update 4 [Internet]

Overactive bladder is defined by the International Continence Society as a syndrome of urinary frequency and urgency, with or without urge incontinence, appearing in the absence of local pathological factors. Treatment of overactive bladder syndrome first requires a clear diagnosis. In patients with incontinence, multiple forms can be present and it is important to determine which form is dominant. Non-pharmacologic, non-surgical treatment consists of behavioral training (prompted voiding, bladder training, pelvic muscle rehabilitation), transcutaneous electrical nerve stimulation, catheterization, and use of absorbent pads. Pharmacologic treatment for overactive bladder syndrome includes darifenacin, flavoxate hydrochloride, hyoscyamine, oxybutynin chloride, tolterodine tartrate, trospium chloride, scopolamine transdermal, and solifenacin succinate. The purpose of this systematic review is to compare the benefits and harms of drugs used to treat overactive bladder syndrome.

Radiotherapy to the pelvis for cancer can damage the bladder in some, leading to late radiation cystitis

This can cause urinary problems including pain, blood in the urine and reduced bladder capacity. A cycle of bleeding, infection and occasionally life‐threatening complications can occur. Options include treating infections, blood transfusion, catheterisation, drugs inserted into the bladder, and surgery. This review found no evidence from trials to determine the effects of non‐surgical treatments for late radiation cystitis, although some drugs inserted into the bladder may be advantageous.

Bladder training for urinary incontinence in adults

Urinary incontinence is the inability to control the leakage of urine and is a common and distressing problem. Urge incontinence is leakage of urine when a person is unable to control the strong desire to pass urine (void). Stress incontinence is the leakage of urine when a person coughs or undertakes physical exertion. Bladder training encourages people to extend the time between voiding so that continence might be regained. This can take months to achieve but may help people who are physically and mentally able to use this method. The review of trials did not find enough rigorous evidence and concluded that more research is needed. The limited evidence available suggests that bladder training may be helpful in treating urinary incontinence but this is not definite.

The role of anticholinergics in men with lower urinary tract symptoms suggestive of benign prostatic hyperplasia: a systematic review and meta-analysis

The authors concluded that anticholinergics appear safe in men with lower urinary tract symptoms suggestive of benign prostatic hypertrophy, but more research is required to evaluate their efficacy. The authors’ cautious conclusions appear to reflect the evidence from a small number of studies, but the poor reporting of review methods means that the reliability of the conclusion is unclear.
