Clinical education

Activities delivered by primary care clinicians and related health care staff (e.g., health educator, social worker, nursing staff) to assist patients in adopting, changing, or maintaining behaviors related to fall risk, including exercise, fall risk reduction, and home hazard checklists. Education/counseling is delivered to individuals or small groups and does not primarily involve group-level interventions outside the primary care setting or more than 8 group sessions.
