Vitamin A supplementation for prophylaxis or therapy in childhood pneumonia: a systematic review of randomized controlled trials

This review concluded that there was no benefit of vitamin A supplementation for the prevention and treatment of childhood community acquired pneumonia. However, due to problems with the conduct and reporting of the review, the author's conclusion may not be reliable.

Do guidelines guide pneumonia practice: a systematic review of interventions and barriers to best practice in the management of community-acquired pneumonia

The authors concluded that interventions which encourage adherence to community-acquired pneumonia guidelines are safe and are associated with improvements in patient and process outcomes. The limited search, inadequate reporting of review methods and quality assessment of the included studies, and the paucity of good quality evidence, make the reliability of such a strong conclusion uncertain.

Evidence behind the 4-hour rule for initiation of antibiotic therapy in community-acquired pneumonia

The authors concluded that observational evidence does not confirm that mortality is decreased by early administration of antibiotics in stable adult patients with community-acquired pneumonia. These conclusions did not appear to sufficiently highlight the poor quality of the evidence, which was derived from a small number of potentially biased and poor-quality observational studies.

The role of corticosteroids in severe community-acquired pneumonia: a systematic review

The authors concluded that available evidence did not support the use of corticosteroids as standard care for patients with severe community-acquired pneumonia. Further research was required. The studies were either very small or uncontrolled. The results of the review were limited by the paucity of good quality data. The conservative conclusion seemed appropriate.

Antibacterial class is not obviously important in outpatient pneumonia: a meta-analysis

This review assessed whether improved clinical and mortality outcomes are achieved for outpatient community acquired pneumonia treated with antibacterials with coverage against atypical organisms. The authors concluded that there was no evidence of superiority for any drug comparison. Given some methodological uncertainties identified in the review process, the extent to which this conclusion was reliable was unclear.

Effect of pneumonia case management on mortality in neonates, infants, and preschool children: a meta-analysis of community-based trials

This review of the community-based case management approach proposed by the World Health Organization for developing countries concluded that intervention reduces mortality in neonates, infants and preschool children and should be part of primary health care. Significant heterogeneity was present for some outcomes, suggesting that the results may not be generalisable.
