A patient with idiopathic Parkinson's disease and "on-off" phenomena is described. A consistent relationship existed between the "on" (dyskinetic) phases and mania and between the "off" (akinetic) phases and depression. The heuristic interest of this case and its indirect support of the dopamine hypothesis of affective disorder are discussed.
