PURPOSE: To provide physicians and nurses with an overview of strategies for descriptive documentation of skin and wound assessments and interventions. TARGET AUDIENCE: This continuing education activity is intended for physicians and nurses with an interest in learning about strategies for documenting skin and wound assessments and interventions. OBJECTIVES: After reading the article and taking the test, the participant should be able to: 1. Describe documentation related to the use of risk assessment and manual assessment tools. 2.
