We present the case of a 77-year-old woman with corticobasal degeneration (CBD) which still is an underdiagnosed neurodegenerative disease. She revealed stiffness and dystonia of the right hand at the disease onset. Brain magnetic resonance imaging (MRI) and single photon emission computed tomography (SPECT-HMPAO) showed changes in the left cerebral hemisphere, opposite to neurological signs. The authors present a detailed differential diagnosis that enabled to several other degenerative disorders of the brain to be excluded. In the present case, symptomatic treatment was ineffective. We emphasize that the diagnosis can be made during life based on clinical signs and neuroradiologic studies such as MRI, SPECT-HMPAO and proton magnetic resonance spectroscopy.
