Instructions for Submitting Registration Forms
A new laboratory, clinical test, research test, or
miscellaneous testing/banking may be registered using either PDF
or MS Word format:

Portable Document Format (PDF)
(requires Adobe©
Acrobat Reader)
- In the 'Select Format' column for the applicable
registration form below, select
. The form will open on your screen.
- Print the form and fill in the information.
- Fax or mail the
completed form to GeneTests; see form for fax and
postal mailing information.
Note: With a dial-up service,
the PDF option is faster than MS Word.

MS Word for Windows Format
(may be submitted as an e-mail
attachment)
- In the 'Select Format' column for the applicable
registration form below, select

- Click 'OK' in the pop-up window. The form will
open in Word on your screen.
- Fill out the form on your screen and save it to
your hard drive.
- E-mail the completed form
as an attachment to
gtlabs@u.washington.edu.
Note: The Word form may also
be printed, then faxed or mailed to GeneTests; see
form for fax and postal mailing
information.

- Miscellaneous testing and banking: Includes banking of DNA/RNA and testing that is not specific to a single disease
