Laboratory Registration

Registration and listing of laboratories are free. Required registration forms (see below for descriptions and examples) are available online.

Required Registration Forms

• Laboratory Registration: completion required for each new laboratory; see completed example.
  
  
• Clinical Test Registration: completion required for each clinical test offered by the laboratory in house; see completed example.
  
  
• Research Test Registration: completion required for each research test offered by the laboratory; see completed example.
  
  
• Laboratory Service Registration; completion required for all non-disease-related testing^*; see completed example.
  
  

* Examples include: genotyping for identity and blood incompatibility; confirmation of mutations identified in research laboratories; DNA banking.

Updating of Registered Laboratory Information

• Laboratories are contacted approximately once a year to review listing information for accuracy.
  
  
• Laboratories may initiate modification of a listing at any time:
  • On the home page select 'Administrative Use;' enter your User Name and Password; select 'Laboratory Contacts.'
