Instructions for Submitting Registration Forms

A new clinic may be registered using either PDF or MS Word format:

1. Portable Document Format (PDF) (requires Adobe^© Acrobat Reader)

   1. In the 'Select Format' column for the applicable registration form below, select . The form will open on your screen.
      
      
   2. Print the form and fill in the information.
      
      
   3. Fax or mail completed form to GeneTests.
      
      

2. MS Word for Windows Format (may be submitted as an e-mail attachment. Note: If you are using a dial-up service, the PDF option will save time.)

   1. In the 'Select Format' column for the applicable registration form below, select .
      
      
   2. Click 'OK' in pop-up window (default is set to open in Word). The form will open on your screen.
      
      
   3. Fill out the form on your screen and save to your hard drive.
      
      
   4. E-mail the completed form as an attachment to gtclinic@u.washington.edu (Subject: Clinic Registration Form). Note: Word form may also be printed and faxed or mailed; see form for fax and postal mailing information.)
