Enrollment Methods:
Every 10th patient admitted to University of Chicago is asked to consent to contacting their PCP as part of a large ongoing study of quality of care. Because we were interested in oversampling frail older patients, those patients that were screened as frail using the Vulnerable Elder Survey-13 during the inpatient interview were also asked to consent to contact their PCP.
