Low Risk	PSA < 10 ng/mL and Gleason biopsy ≤ 6 and

Intermediate Risk	PSA 10 – 20 ng/mL or Gleason biopsy 7 or
	1992 AJCC T2_b
High Risk	PSA > 20 ng/mL or Gleason biopsy ≥ 8 or
	1992 AJCC ≥ T2_c
